The following forms can be printed and mailed, faxed or returned to the library. They can also be e-mailed to the director at ltaylor@shawls.lib.il.us.

Facilities Use Request (print version)
- The meeting room is available for community use by civic groups, organizations, and businesses and is available on days the Library is open. Reservations for use of the facilities must be made at least two (2) days in advance with the Library staff and must be approved by the Library Director.

- Your tax-deductible contributions to the Mt. Carmel Public Library Foundation will ensure continued excellence in books, equipment, and programming.

Adopt-A-Book Form
- The Mt. Carmel Public Library’s year round Adopt-A-Book program is a rewarding opportunity to give a gift that can be opened again and again. Anniversaries, birthdays, graduations, in remembrance of a friend or relative, as a commemorative for special occasions are the perfect reasons to adopt a book.

5K registration form and flier
- The 5th Annual 5K Run/Walk will start at 8am on Saturday, June 11, 2011. Registration is from 7:15-7:45. The race begins at Mt. Carmel Public Library and follows a route near the scenic Wabash River, ending back at the library.
